The Transition Coordinator plays a vital role in the Senior Living Division at National Church Residences, serving as a liaison between healthcare and housing professionals. This position is focused on assisting elderly individuals and their families in navigating supportive services and resources, ensuring they receive the necessary care and benefits. The role requires a strong understanding of community resources and a commitment to providing quality customer service to enhance the living conditions of seniors.

Responsibilities

• Serve as a liaison between healthcare and housing professionals, including service coordinators and discharge planners.
• Assist individuals in arranging services based on their needs and preferences, such as locating housing and accessing community resources.
• Provide quality customer service to residents, addressing questions and concerns, and problem-solving daily needs.
• Work closely with managers of all service lines to coordinate care and services for individuals.
• Purchase materials and supplies necessary for living arrangements.
• Assist individuals in ensuring they receive entitled benefits.

Requirements

• High school diploma or GED required; Bachelor's Degree in social work or a related field preferred.
• Minimum of 1 year of experience in social service delivery with elderly and/or family populations; 2-4 years preferred.
• Demonstrated working knowledge of supportive services and resources in the area served.
