About the position

The Transition Coordinator position at Waymakers involves providing in-home mental health rehabilitation services to youth involved in the Juvenile Justice system. The role focuses on assisting youth in developing essential life skills, promoting their success and well-being, and ensuring they can navigate their environments effectively. This position requires a commitment to after-hours availability for emergencies and a strong dedication to helping youth achieve their goals.

Responsibilities

  • Provide in-home mental health rehabilitation services to youth involved in the Juvenile Justice system.
  • Assist youth in acquiring healthy social skills, positive decision-making skills, school completion, career readiness, independent living skills, and conflict resolution skills.
  • Work directly with consumers aged 12-25, assisting them with daily activities related to their Care Plan.
  • Document any services provided daily.
  • Maintain a 'whatever it takes' approach to resolving challenges and encourage consumers to focus on meeting their goals.

Requirements

  • Bachelor's degree in psychology, social work, criminology, or a related discipline.
  • 1-2 years of experience in the field.
  • Ability to establish rapport and make accurate assessments of a youth and family's needs.
  • Assist youth and families in practicing coping skills taught.
  • Good written and communication skills.
  • Demonstrates the ability to work with a multicultural and diverse population with sensitivity and respect.
  • Must be available for after-hours emergencies with 24/7 capability to respond via agency-issued cell phone.
