About the position
The Transition Coordinator at CareStar, Inc. plays a crucial role in assisting individuals with disabilities in transitioning to community living. This part-time position involves collaborating with healthcare teams and service providers to ensure that individuals receive the necessary support and resources to live independently. The role emphasizes advocacy, problem-solving, and effective communication to empower individuals in managing their healthcare and personal needs.
Responsibilities
- Act as point-of-contact to assist individuals in living in their home and/or community.
- Assist individuals with linkages to service providers and community resources as needed.
- Identify barriers to compliance with individuals' healthcare regimes, appointments, and screenings.
- Work with individuals to create solutions to identified barriers and empower them to manage their healthcare contacts and appointments.
- Communicate and collaborate with the individual's healthcare team, including facility Social Workers and Waiver Case Managers.
Requirements
- High school diploma or GED.
- Minimum of 12 months of experience delivering services in a healthcare, behavioral health, or social services setting.
- Experience advocating on behalf of individuals with chronic illness, mental health and/or substance use disorders, and physical disabilities.
