About the position

The Transition Coordinator role focuses on facilitating the implementation of a life skills curriculum for youth transitioning back into the community. This position involves working closely with various agencies to support students in obtaining necessary documentation and resources for successful reintegration, while also participating in administrative committees to ensure effective reentry plans.

Responsibilities

  • Facilitate the implementation of a life skills curriculum, titled Next Steps.
  • Utilize the Transition Coordinator Handbook to standardize transition practices.
  • Evaluate the five 'Transition Consultations' for every student under the guidance of an assigned teacher.
  • Participate weekly in Administrative Release Committees (ARC) to confirm and reinforce student reentry plans.
  • Conduct follow-up phone calls to released youth at 1, 3, 6, 9, and 12 months post-release.
  • Represent the Indiana Department of Correction- Division of Youth Services at conferences and meetings.
  • Assist in developing partnerships with Indiana's Department of Workforce Development, HIRE, and Indiana University's HOPE Mentoring.
  • Assist in developing individualized Education Learning Plans for each youth upon entry at the facility.
  • Work with state, county, and private agencies to provide re-entry services for youth transitioning back into the community.
  • Assist students in obtaining social security cards, state identification, driver's licenses, birth certificates, and additional documentation.
  • Support students in transitioning back to public schools and provide information on employment, apprenticeship programs, and military service.

Requirements

  • High school diploma or equivalent (preferred).
  • 2 years of experience in training, coaching, or guiding youth (required).
  • 2 years of experience assisting students with obtaining social security cards (required).
  • 1 year of experience assisting students with obtaining driver's licenses (required).
  • 1 year of experience assisting students with obtaining birth certificates (required).
  • 1 year of experience assisting students with employment (required).
  • 1 year of experience communicating effectively in a team environment (required).
