About the position

The Transition Coordinator plays a vital role in supporting students with special educational needs, focusing on their academic, emotional, and social development. This position involves collaborating with school teams and families to provide personalized guidance and implement evidence-based instructional models. The coordinator will work with students aged 14 and up to prepare them for successful transitions from high school to post-secondary opportunities, including job readiness and life skills training.

Responsibilities

  • Diagnose and address learning, social, and emotional skill gaps in students.
  • Collaborate with school teams and families to provide personalized guidance.
  • Deliver evidence-based learning and instructional models, including positive behavioral interventions.
  • Provide academic, emotional, and physical services for students requiring additional support.
  • Help students prepare for the transition from high school to post-secondary opportunities.
  • Work with students on job and life skills, community-based instruction, and college readiness.
  • Develop and maintain individualized skill-appropriate lesson plans.
  • Implement behavior management and intervention strategies, including crisis management.
  • Maintain student documents such as IEPs and behavior intervention plans.
  • Drive, record, and track student growth and progress toward goals.
  • Supervise and monitor students within the school.

Requirements

  • Bachelor's degree from an accredited institution of higher learning required.
  • Valid teaching credential appropriate to the role preferred (secondary special education).
  • Experience in teaching and special education settings.

Nice-to-haves

  • Experience with job corps programs or post-secondary academic institutions.
  • Training in Serve-Safe or Pro-Start (culinary arts).
  • Commitment to learning and implementing feedback.
