About the position

The Transition Coordinator (TC) at Mastery High School of Camden is responsible for overseeing post-secondary transition planning and services for students with special needs. The TC collaborates with case managers and the post-secondary team to create individualized transition plans, ensuring that students are prepared for various post-secondary pathways. The role involves a significant focus on early transition planning for students in grades 7-11 and implementing activities for students in grades 12 and above, including family engagement and community partnerships.

Responsibilities

  • Ensure design and implementation of Special Education Transition services as outlined by Federal and State regulations.
  • Support Case Managers/IEP Teams in developing coordinated Transition Plans that are individualized and student-centered.
  • Align each IEP with the Transition Plan for each student, including goals and related services.
  • Develop and maintain relationships with community agencies focused on transition practices.
  • Design and implement methods to assess and evaluate the effectiveness of transition programming and partnerships.
  • Oversee transition assessments, goal creation, and services in the IEP writing process.
  • Review and analyze student data monthly with the APSS to maximize student learning.
  • Ensure course enrollment for students with disabilities aligns with transition goals.
  • Participate in IEP Team Meetings as needed.
  • Teach transition-related skills to students, such as resume writing and interview techniques.
  • Promote Evidence-Based Practices in secondary transition through professional development for staff.
  • Stay updated on transition guidance at local, state, and federal levels.
  • Submit accurate data to state and federal agencies.
  • Coordinate post-school outcomes tracking and support for Early Alumni.
  • Oversee 18-21 transition services if available.

Requirements

  • Bachelor's degree (Master's preferred) and Special Education Certificate.
  • Minimum of three years' experience in successful Transition Planning.
  • Five years of urban education experience is desired.
  • Experience working with special education students with mild, moderate, and profound disabilities.
  • Strong writing skills and ability to communicate effectively with diverse stakeholders.

Nice-to-haves

  • Experience and knowledge of Best Practice Transition Services.
  • Ability to work well in a team and across teams.
