About the position
The Transitional Care Associate plays a crucial role in facilitating the smooth transition of patients from acute care settings to home health or other post-acute services. This position involves coordinating discharge logistics, ensuring that all necessary information and services are in place for patients upon their discharge from the hospital. The role requires effective communication and collaboration with various healthcare teams and external agencies to support patient care continuity.
Responsibilities
- Coordinate the patient's transition into or out of a care setting and obtain appropriate services and benefits as directed by the care coordination team.
- Manage the logistics of discharge planning for individual patients, including arranging transportation and durable medical equipment (DME).
- Document all interventions in the patient medical record accurately and timely, including elements of the discharge plan.
- Promote collaborative relationships with team members, vendors, and community resources.
- Secure community resources/information as needed and perform other related tasks.
Requirements
- High school diploma/GED or equivalent working knowledge.
- Certification for BLS is required for acute-care settings where direct patient care is provided.
- One year of experience in healthcare as a Nursing Assistant, Medical Assistant, Health Unit Coordinator, Patient Care Technician, etc.
- Effective communication and customer service skills, human relation skills, and time management skills.
