About the position

The Transitional Care Manager/Case Manager (RN) at Orlando Health & Rehabilitation Center is responsible for overseeing the care of patients transitioning from hospital to home or other care settings. This role involves coordinating with healthcare professionals to ensure that patients receive appropriate post-acute and rehabilitative services, while also providing support and guidance to patients and their families throughout the care process.

Responsibilities

• Coordinate patient care transitions from hospital to home or other care settings.
• Collaborate with healthcare professionals to develop and implement care plans.
• Monitor patient progress and adjust care plans as necessary.
• Provide education and support to patients and their families regarding care options and resources.
• Ensure compliance with healthcare regulations and standards.

Requirements

• Current FL state license as an RN
• Bachelor's degree in nursing
• 2-3 years of experience in Case Management
• Experience in a Skilled Nursing environment preferred.
