About the position

The Transitional Healthcare Coordinator is responsible for coordinating and overseeing daily activities related to the Integrated Reentry and Correctional Support (IRACS) program. This role involves managing reentry and treatment coordination, ensuring quality control, and acting as a liaison between various stakeholders. The coordinator will also develop training programs, manage grant deliverables, and support local IRACS program sites in their operations.

Responsibilities

  • Create, support, complete, and oversee reentry/resource navigation coordination and training.
  • Develop and manage reentry treatment coordination solutions for local IRACS program teams.
  • Provide quality control and oversight of the IRACS reentry and navigation team members.
  • Manage various IRACS grant deliverables and meet funding requirements.
  • Develop trainings, oversight QI metrics, and fidelity reviews for IRACS reentry teams.
  • Act as a liaison for IFS to all MHAI partners and uphold organizational values.
  • Support local IRACS program sites in daily operations related to recovery planning and documentation procedures.
  • Coordinate local and supervision activities for IRACS reentry efforts.
  • Manage databases, websites, and reports to ensure accurate information dissemination.
  • Lead reentry/navigation training and development.

Requirements

  • Valid driver's license and reliable personal transportation for travel.
  • At least three years of experience working with treatment providers.
  • At least three years of experience as a peer support in Indiana.
  • Strong understanding of behavioral health services.
  • Ability to work remotely and in-office as projects require.
  • Reliable access to stable high-speed internet and personal smartphone.
  • No conviction for sexual assault, domestic violence, child abuse, or related offenses.
  • Flexible schedule with daytime availability for some aspects of the role.

Nice-to-haves

  • CAPRC II credential; CHW/CRS, NCPS, or NCPRSS credential required within 60 days of hiring.
  • Previous peer supervisory experience.
  • CSPR certification through ICAADA or Peer Recovery Supervision training within 60 days of hiring.
  • Experience providing direct peer support services.
  • Experience with creation, implementation, and sustainment of peer programming.
  • Knowledge of local/statewide recovery community and resources.
  • Strong cultural literacy; bilingual preferred.
