About the position

The Transitional Healthcare Facilitator is a part-time role responsible for developing and implementing transitional healthcare plans for incarcerated individuals preparing for release. This position involves coordination with the Indiana Department of Correction and various state and local agencies to ensure effective pre-discharge planning and the provision of necessary services and resources for re-entering society.

Responsibilities

  • Develop and implement transitional healthcare plans for incarcerated individuals scheduled for release.
  • Coordinate with the Indiana Department of Correction and other agencies for pre-discharge planning.
  • Ensure that individuals re-entering society receive necessary services and resources.

Requirements

  • Bachelor's degree in psychology, social work, counseling, or a related field from an accredited institution.
  • Two years of experience working in a correctional setting.
  • Experience with community and state programs and services.
  • Two years of experience in re-entry services or release planning preferred.
  • Current Purified Protein Derivative (PPD) documentation for tuberculosis screening.
  • Certification in Cardio-Pulmonary Resuscitation (CPR) and Automated External Defibrillator (AED) use.
