About the position

The Transitional Healthcare Facilitator is responsible for developing and implementing transitional healthcare plans for incarcerated individuals scheduled for release at the Putnamville Correctional Facility in Greencastle, Indiana. This role involves coordinating with the Indiana Department of Correction and other agencies to ensure effective pre-discharge planning and the provision of necessary services and resources for individuals re-entering society.

Responsibilities

• Develop and implement transitional healthcare plans for incarcerated individuals scheduled for release.
• Coordinate with the Indiana Department of Correction and other state and local agencies.
• Ensure pre-discharge planning and preparation is accomplished.
• Provide necessary services and resources to individuals re-entering society.

Requirements

• Bachelor's degree in psychology, social work, counseling, or a related field from an accredited institution.
• Two years of experience working in a correctional setting.
• Experience with/knowledge of community and state programs and services.
• Two years of experience in re-entry services or release planning preferred.
• Current Purified Protein Derivative (PPD) documentation for tuberculosis screening.
• Certification in Cardio-Pulmonary Resuscitation (CPR) and Automated External Defibrillator (AED) use.
