About the position

The Transitional Housing Program Case Manager at 360 Youth Services is responsible for supporting youth aged 18-24 who are experiencing homelessness. This role involves building positive relationships with residents, developing individual case plans, and providing intensive case management services to help youth achieve self-sufficiency through education, employment, and life skills development. The Case Manager will work collaboratively with the housing team and community partners, ensuring a trauma-informed approach to care.

Responsibilities

  • Build positive and supportive relationships with residents using a trauma-informed approach.
  • Develop individual program case plans for all residents on caseload.
  • Maintain communication with housing team members through written notes, oral communications, and weekly meetings.
  • Provide weekly intensive case management services focusing on education enrollment, vocational training, health and wellness appointments, and recreational needs.
  • Coordinate services with community resources and evaluate residents' progress.
  • Advocate for clients enrolling in eligible public benefits and health care.
  • Support residents in building a budget and learning personal finance.
  • Complete all required documentation including case notes, case plans, intake and discharge summaries, contracts, and releases.
  • Assist residents in finding permanent affordable housing prior to discharge.
  • Participate in all discharge planning, aftercare follow-up, and advocacy.
  • Share on-call 24/7 duties with Program Managers and Case Managers for emergencies/crisis.

Requirements

  • Bachelor's degree in social work or related human services field and one-year experience or equivalent work experience preferred.
  • Demonstrated ability and willingness to provide agency designated services independently.
  • Proficiency in Microsoft Office Suite required.
  • Ability to complete a background check.
  • Valid driver's license and proof of personal automobile insurance as required by Illinois State Law.

  • Experience in affirming care for LGBTQ+ young people and commitment to affirming care with BIPOC populations.
