About the position

The Transitional Management Services Qualified Professional (QP) plays a crucial role in providing clinically-based supervision and guidance to staff and individuals with mental health diagnoses and/or substance use disorders. This position focuses on enhancing the quality of life for individuals through person-centered services, ensuring effective communication, and fostering community connections. The role emphasizes professional growth, teamwork, and the development of future leaders within the organization.

Responsibilities

  • Provide initial and ongoing assessment of individuals receiving services.
  • Evaluate the effectiveness of the Person-Centered Plan (PCP) and monitor progress.
  • Participate in team meetings and case reviews with other providers and natural supports.
  • Identify opportunities for community connections and enhance natural support networks.
  • Educate individuals on available options and access opportunities.
  • Establish mutual relationships using effective communication skills with individuals and stakeholders.
  • Ensure service requests align with eligibility and discharge criteria.
  • Conduct clinical authorization and manage billing systems effectively.
  • Prepare and submit required reports, assessments, and documentation.
  • Review and sign off on clinical documentation as required.
  • Identify methods to utilize existing community resources for service provision.
  • Continuously apply person-centered approaches in interactions with consumers.
  • Provide direction to staff regarding program implementation and participant needs.
  • Monitor staff performance and implementation of responsibilities through unannounced visits.
  • Ensure staff training on plans and program components is adequate.
  • Assist in designing and presenting agency-wide training programs.
  • Maintain certification in agency, state, and federal training requirements.
  • Demonstrate knowledge of emergency procedures and crisis services.
  • Participate in on-call coverage as scheduled.
  • Collaborate with supervisors to assess and enhance service delivery.
  • Comply with agency policies and state and federal regulations.

Requirements

  • Bachelor's degree in Human Services or related field (required).
  • Master's degree in Human Services (preferred).
  • Valid Qualified Professional (QP) certification.
  • Valid driver's license.
  • At least 2 years of experience working with adults with mental health diagnoses and/or substance use disorders.

Nice-to-haves

  • Experience in clinical documentation and billing systems.
  • Knowledge of community resources for mental health and substance use services.
