About The Position

The Transitions of Care Pharmacist at Hackensack Meridian Health plays a crucial role in optimizing medication therapy management to enhance clinical, economic, and humanistic outcomes for patients. This position involves collaboration with healthcare providers, conducting comprehensive medication reviews, and ensuring effective medication reconciliation during transitions of care.

Requirements

  • BS in Pharmacy or PharmD from an accredited college.
  • Two years of work experience in a clinical setting.
  • Strong clinical therapeutic skills and knowledge of medication dosing in geriatric and adult populations.
  • Self-directed with excellent organizational skills.

Nice To Haves

  • Pharmacy Residency (PGY-1) preferred.
  • Experience with the 5 Core Elements of Medication Therapy Management Services.
  • Effective communication skills, both verbal and written.
  • Excellent listening skills and empathy in patient interactions.
  • Ability to maintain professionalism and cultural competency.

Responsibilities

  • Conducts Comprehensive Medication Reviews and Targeted Interventions to assess appropriateness of all medications.
  • Engages in active patient-centered medication therapy management through transitional care including medication reconciliation services and discharge assistance.
  • Assists with managing medication costs through coordination of benefits and formulary adherence.
  • Consults with prescribers and the interdisciplinary care team to identify patient-specific therapy goals.
  • Provides interdisciplinary support and communicates clinical recommendations to prescribers and the care team.
  • Coordinates medication reconciliation across the continuum of care to improve clinical outcomes.
  • Attends interdisciplinary rounds to assist with medication-related plans for discharge.
  • Identifies additional therapies needed or therapies no longer needed.
  • Assists with staffing as deemed necessary by the Director of Pharmacy.
  • Modifies and delivers care specific to the needs of the patient population served.
