About the position
The Transplant Coordinator for BMT & Cell Therapy is responsible for managing the care of solid organ transplant patients, ensuring continuity of care between inpatient and outpatient settings. This role involves evaluating potential transplant recipients and donors, monitoring postoperative outcomes, and providing education and support throughout the transplant process. The position is hybrid, allowing for both remote and in-office work, and operates Monday to Friday with no weekends or holidays.
Responsibilities
- Manages the health care of transplant patients and donors.
- Coordinates care with physicians and health care team.
- Provides education throughout continuum of care.
- Documents patient assessment and diagnosis, plan of care, implementation and evaluation of care and teaching on assigned patients.
- Collaborates with interdisciplinary team to identify and resolve problems in patient care management.
Requirements
- Nursing Diploma/Associate's
- 2-5 years of nursing experience
- Valid RN License
Nice-to-haves
- Bachelor's Degree in Nursing
- 5-10 years of nursing experience
- Certification as Clinical Transplant Coordinator
