About the position

The Heart Transplant Coordinator at Duke University Hospital is responsible for providing advanced nursing care to heart transplant patients throughout all phases of the transplant process. This role involves working collaboratively with a multidisciplinary team to ensure exemplary patient experiences and outcomes, coordinating care from pre-transplant assessments to post-transplant follow-up. The coordinator will also engage in patient education, clinical practice, and research utilization, contributing to the overall excellence of the Heart Transplant Program.

Responsibilities

  • Provide advanced nursing care for patients before and after solid-organ heart transplantation 24/7.
  • Coordinate evaluation processes for pre-transplant patients to ensure effective use of resources.
  • Assess transplant organ function and general health status, determining the need for additional interventions.
  • Facilitate scheduling of diagnostic studies or procedures with fiscal responsibility.
  • Participate in developing and implementing protocols and standards of care for transplant patients.
  • Assist nursing staff in assessing, planning, implementing, and evaluating patient care.
  • Monitor routine care processes and outcomes to identify quality improvement needs.
  • Provide on-call coverage and coordinate activities prior to transplant.
  • Coordinate the transplantation process when a donor is available, ensuring communication among all parties involved.
  • Complete necessary documentation according to UNOS guidelines.
  • Communicate effectively with multidisciplinary team members and patients/families regarding care issues.
  • Collaborate in coordinating discharge plans for patients pre-transplant or post-transplant.
  • Communicate with insurer case managers as needed.
  • Refer patients/families to health system and community resources as appropriate.
  • Maintain current patient transplant records to facilitate communication.
  • Plan nursing care and medical procedures with fiscal responsibility.

Requirements

  • BSN required; MSN preferred.
  • Minimum of 5 years work experience as a bedside Registered Nurse, with at least 2 years in transplant or critical care.
  • Previous experience in a cardiovascular unit is highly preferred.
  • Current or compact RN licensure in North Carolina required.
  • BLS certification required.

Nice-to-haves

  • Experience in a transplant program or with organ donation processes.
  • Advanced certifications in nursing or transplant care.
