About the position

The Transplant Nurse Coordinator for Acute Leukemia is responsible for planning, directing, and providing specialized nursing care for individuals with end-stage organ disease who are designated for solid organ transplants. This role emphasizes continuity and coordination of patient care, education initiatives, and collaboration with multidisciplinary teams and community resources.

Responsibilities

  • Facilitate living donation and transplantation, coordinating care for living donors.
  • Coordinate care for transplant recipients in the outpatient setting to achieve optimal rehabilitation.
  • Identify and respond to the educational, psychosocial, and economic needs of recipients and their families post-transplant.
  • Communicate lab data and diagnostic results to transplant physicians and healthcare providers.
  • Implement changes in drug/treatment therapy as ordered by physicians.
  • Arrange consultations, diagnostic procedures, and hospitalizations as needed.
  • Maintain communication with patients, referring physicians, payers, and transplant team members.
  • Monitor and coordinate care for candidates awaiting organ transplantation.
  • Develop and institute teaching plans for transplant candidates and their families.
  • Maintain comprehensive documentation of candidate progress and ensure compliance with OPTN/UNOS regulations.
  • Educate recipients and families about post-transplant care and responsibilities.
  • Act as a liaison between donor and recipient transplant teams, facilitating communication and coordination.
  • Ensure compliance with OPTN/UNOS policies and listing requirements.

Requirements

  • Bachelor's Degree in Nursing.
  • Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing.
  • Basic Cardiac Life Support (BLS) certification or obtained within 14 days.
  • 2 or more years of acute clinical experience.

Nice-to-haves

  • Current active multistate RN license in good standing or proof of application for multistate RN license.
