About the position

The Transportation Coordinator/Scheduler is responsible for overseeing and coordinating transportation services for residents in a skilled nursing facility. This role ensures that residents are transported safely and timely to medical appointments, activities, and other essential needs, while also assisting with administrative duties related to medical records.

Responsibilities

• Coordinate and schedule transportation for residents to medical appointments, therapy, and outside activities.
• Develop and maintain a detailed schedule/calendar for drivers and transportation vehicles.
• Communicate appointment times and schedules with residents, families, facility staff, and drivers.
• Collaborate with nursing, social services, and admissions teams to prioritize transportation needs.
• Maintain accurate, up-to-date records on trip sheets, fuel purchases, incident and accident reports, and vehicle condition reports.
• Be available as a chaperone with patients during transportation as needed.

Requirements

• Experience working in a Skilled Nursing Facility (SNF), Assisted Living, or healthcare environment.
• High School Diploma or equivalent.
• Must be a licensed certified nursing assistant in this state.
• Must present a professional appearance and reputation to represent Avamere and this facility.
• Maintain confidentiality of all resident care information in accordance with HIPAA guidelines.
