About the position

The Trauma Program Continuum of Care Coordinator at Integris Health is responsible for supporting injured patients throughout their care journey and facilitating Trauma Program initiatives. This role involves collaboration with various departments to assess patient needs, develop resources, and promote teamwork among stakeholders. The coordinator will also engage in research, audits, and educational activities related to trauma care, ensuring compliance with established standards.

Responsibilities

  • Collaborates with Trauma Program Leadership and departments to assess trauma patient needs across the continuum of care.
  • Develops and updates trauma continuum of care resources for patients and families.
  • Promotes teamwork and positive relationships with internal and external stakeholders.
  • Maintains trauma patient resource material and continually researches advancements in trauma care.
  • Participates in trauma research related to the continuum of care.
  • Completes audits as assigned by the Trauma Program Manager.
  • Assists in coordinating trauma patient follow-up related to continuum of care.
  • Engages in injury prevention, outreach, education, and performance improvement activities as assigned.
  • Maintains records for ACS trauma verification.
  • Participates in Trauma Operations Committee, Trauma M&M, and Trauma Process Improvement meetings.

Requirements

  • Bachelor's Degree in Public Health or Business Administration; Master's preferred.
  • Graduate of an Accredited School of Nursing; Bachelor's required, Master's preferred.
  • Minimum of five years of experience in Continuum of Care, public health, counseling, or motivational interviewing; OR three years in a healthcare or public health setting.
  • Current Oklahoma RN Licensure with the State Board of Nurses (for RN applicants).
  • Basic Life Support certification to be obtained within 60 days of hire.
