About the position

The Trauma Program Continuum of Care Coordinator at Integris Health is responsible for supporting injured patients throughout their care journey and facilitating Trauma Program initiatives. This role involves collaboration with various departments to assess patient needs, develop resources, and promote effective communication among stakeholders. The coordinator will also engage in research, audits, and educational activities related to trauma care, ensuring compliance with established standards.

Responsibilities

  • Support injured patients and their needs throughout the Continuum of Care.
  • Collaborate with Trauma Program Leadership and other departments to establish trauma patient needs.
  • Develop and update trauma continuum of care resources for patients and families.
  • Promote teamwork and positive relationships with internal and external stakeholders.
  • Maintain trauma patient resource material.
  • Research trauma continuum of care advances and share information with the Trauma Program.
  • Participate in trauma research related to the continuum of care.
  • Complete audits as assigned by the Trauma Program Manager.
  • Assist in coordinating trauma patient follow-up as assigned.
  • Assist in injury prevention, outreach, education, and performance improvement activities.
  • Maintain records for ACS trauma verification.
  • Participate in Trauma Operations Committee and related meetings.

Requirements

  • Bachelor's Degree in Public Health, Business Administration, or Nursing required; Master's preferred.
  • Minimum of five years of experience in Continuum of Care, public health, or counseling; or three years in a healthcare setting.
  • Current Oklahoma RN Licensure for RN applicants.
  • Basic Life Support certification to be obtained within 60 days of hire.
  • Knowledge of principles and practices of trauma continuum of care.
  • Strong time management and organizational skills.
  • Excellent oral and written communication skills.

Nice-to-haves

  • Experience in motivational interviewing.
  • Experience in trauma care or public health settings.
