About the position

The Trauma Program Social Worker plays a crucial role in providing comprehensive support to traumatically injured patients across all age groups in an acute care setting. This position involves a range of responsibilities including counseling, needs assessments, discharge planning, and coordination of outpatient services to address the health-related needs of patients. The social worker collaborates with various healthcare professionals to ensure effective patient care and support.

Responsibilities

  • Provide information and referral services to patients and families.
  • Conduct counseling sessions for patients and their families.
  • Perform needs assessments and screenings for patients.
  • Develop and implement discharge plans for patients.
  • Coordinate outpatient services for traumatically injured patients.
  • Monitor and evaluate the effectiveness of care provided to patients.
  • Educate patients and families about available resources and services.
  • Collaborate with healthcare team members including physicians, nursing staff, and community agencies.

Requirements

  • Master of Social Work (MSW or MSSW) is required.
  • Licensed Clinical Social Worker (LCSW) preferred; if not, must begin supervision within 12 months of hire.
  • Certified Social Worker (CSW) for Kentucky is required.
  • Experience in hospital or healthcare social work is preferred.
