Responsibilities

• Assists individuals in skill-building activities as defined by the Staff Action Plan.
• Helps participants gain and maintain independence in daily living skills.
• Provides respite to caregivers in home settings.
• Facilitates community involvement for participants.
• Teaches social skills in home and community settings.
• Participates in staff meetings and training sessions.
• Submits necessary billing and program documentation, ensuring compliance with EVV standards.
• Safely transports individuals to activities and appointments in a personal vehicle.
• Complies with all applicable regulations and agency policies.

Requirements

• Must be at least 18 years of age.
• High School Diploma or GED is required.
• Valid Driver's License that meets agency policy is necessary.
• Ability to transport individuals in a personal vehicle that meets agency requirements.
• Proficiency in using a personal device (cell phone, tablet, etc.) for documentation purposes.
• Ability to meet physical demands of the job, including standing, walking, sitting, bending, squatting, kneeling, and climbing.
