This part-time position requires flexibility in working hours, including days, evenings, and weekends, and involves travel to various locations to deliver services.

Responsibilities

• Provides instruction and supervision to individuals in Community Services Programs.
• Assists participants in skill-building activities as defined by the Staff Action Plan.
• Helps individuals gain and maintain independence in daily living skills.
• Offers respite to caregivers in home settings.
• Facilitates community involvement and social skills development.
• Participates in staff meetings and in-service training.
• Safely transports individuals to activities and appointments using a personal vehicle.
• Submits necessary billing and program documentation, ensuring compliance with regulations.

Requirements

• Must be at least 18 years of age.
• High School Diploma or GED is required.
• Valid Driver's License that meets agency policy is necessary.
• Ability to transport individuals in a personal vehicle that meets agency requirements.
• Ability to use a personal device (cell phone, tablet, etc.) for documentation purposes.
• Must meet physical requirements including standing, walking, sitting, bending, squatting, kneeling, and climbing.
