About the position

The Travel Nurse Practitioner at Monogram Health plays a vital role in delivering personalized and compassionate care to patients with chronic kidney disease (CKD) and end-stage renal disease (ESRD). This position involves traveling nationwide to conduct in-home assessments and provide care to underserved patients, ensuring they receive the necessary support to improve their quality of life and manage their health effectively. The role emphasizes the importance of patient education, coordination of care, and maintaining high standards of documentation and quality of care.

Responsibilities

  • Travel nationwide to conduct in-home assessments, including comprehensive annual wellness exams and focused screenings.
  • Counsel and educate patients and families about available benefits and programs for healthier living.
  • Document patient information accurately, including chief complaints, diagnoses, medical history, and treatment plans.
  • Coordinate care with primary care providers, specialists, and ancillary services.
  • Complete all documentation and paperwork in a timely manner.
  • Maintain quality of care standards as defined by the practice.
  • Deliver evidence-based, timely care to reduce avoidable hospitalizations and maximize patient satisfaction.
  • Prescribe medications, order tests, and collaborate with patients' Monogram physicians.
  • Facilitate closing gaps in care by educating patients about preventive monitoring and scheduling diagnostic testing.
  • Participate in integrated care team meetings and follow up on patients' health status.

Requirements

  • Active and unrestricted Registered Nurse and Nurse Practitioner Board certification (ANCC/AANP).
  • Current and unrestricted DEA certificate or eligibility.
  • Ability to travel extensively by air and utilize rental vehicles.
  • Ability to work autonomously without direct supervision.
  • Access to transportation, a valid driver's license, and car insurance.
  • Proficiency with medical instruments and equipment required for the role.
  • Knowledge of computer-based data management programs and medical records systems.
  • Effective communication skills in verbal and written form with various stakeholders.
  • Understanding of federal and state regulations including HIPAA and OSHA.
  • A minimum of 2 years of direct patient care experience.

  • Experience in chronic kidney disease or end-stage renal disease care.
  • Familiarity with telehealth practices and technologies.
