Responsibilities

• Coordinate patient care and manage discharge planning.
• Utilize InterQual Criteria and Milliman Guidelines for patient assessments.
• Ensure compliance with admission criteria for various units.
• Work collaboratively with healthcare teams to facilitate patient transitions.
• Document care coordination activities and patient progress.

Requirements

• 2 years minimum experience in a healthcare setting.
• BLS certification required (AHA or ARC).
• ACLS certification required for RN/LPN working in ICU, CCU, ED, PACU, and Tele units.
• PALS certification required for RN/LPNs working in ED or Pediatrics.
• State License verification required prior to start.
• NIH Stroke Certification as required by unit description or for MS/Tele and ICU.
• Medication Test/Pharmacology Competency Exam required within 3 years of start, must score 80% or higher.
• Color Blind Testing required (online or in-person).
• Dysrhythmia/EKG Competency Exam required for Telemetry, Critical Care, ED units within 3 years and 80% passing score.

Nice-to-haves

• Previous charge experience preferred.
