Responsibilities
- Manage patient cases in hospice settings.
- Coordinate care with healthcare teams and family members.
- Develop and implement individualized care plans for patients.
- Provide emotional support and counseling to patients and their families.
- Ensure compliance with healthcare regulations and standards.
- Document patient progress and care provided in medical records.
- Participate in interdisciplinary team meetings to discuss patient care.
- Educate patients and families about hospice services and resources.
Requirements
- Current RN license in the state of Wisconsin.
- Experience in hospice or palliative care.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate and patient-centered approach to care.
- Basic computer skills for documentation and communication.
Nice-to-haves
- Certification in hospice and palliative care (CHPN).
- Bilingual skills to communicate with diverse patient populations.
