Responsibilities

  • Manage patient cases in hospice settings.
  • Coordinate care with healthcare teams and family members.
  • Develop and implement individualized care plans for patients.
  • Provide emotional support and counseling to patients and their families.
  • Ensure compliance with healthcare regulations and standards.
  • Document patient progress and care provided in medical records.
  • Participate in interdisciplinary team meetings to discuss patient care.
  • Educate patients and families about hospice services and resources.

Requirements

  • Current RN license in the state of Wisconsin.
  • Experience in hospice or palliative care.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Compassionate and patient-centered approach to care.
  • Basic computer skills for documentation and communication.

Nice-to-haves

  • Certification in hospice and palliative care (CHPN).
