About the position

The Director of Case Management is responsible for overseeing and coordinating the activities of the case management department within a healthcare facility. This role ensures compliance with organizational policies and regulatory standards while leading the integration of patient care and discharge planning processes. The Director is accountable for daily operations, engaging interdisciplinary teams, and optimizing clinical resource utilization to achieve established outcomes.

Responsibilities

• Direct and evaluate departmental operations, including case management model and staffing.
• Allocate resources to effectively staff the department and meet productivity and quality goals.
• Oversee core functions and practices of case management.
• Ensure adherence to care coordination and discharge planning processes, ensuring timeliness and quality.
• Work with the Facility CFO and Division CM Leader to achieve established goals and expectations.
• Assess and improve departmental performance by evaluating operational processes and implementing performance improvement activities.
• Establish working relationships with key stakeholders including CMO, CNO, and ancillary service leaders.
• Perform other duties as assigned.

Requirements

• Bachelor's degree in Nursing required.
• Master's degree in Nursing, Health Administration, or Business Administration preferred.
• 3+ years of overall acute care experience.
• 2+ years in case management leadership in an acute care setting.
• Previous HCA experience highly preferred.
