Responsibilities

• Provide physical therapy services to patients in a home health setting.
• Assess patient needs and develop individualized treatment plans.
• Implement therapeutic interventions to improve patient mobility and function.
• Document patient progress and adjust treatment plans as necessary.
• Collaborate with healthcare team members to ensure comprehensive patient care.

Requirements

• Valid Physical Therapist license in the state of West Virginia.
• Experience in home health therapy or rehabilitation settings.
• Ability to work independently and manage a caseload effectively.
• Strong communication and interpersonal skills.

Nice-to-haves
