About the position

Epic Travel Staffing is seeking a full-time Travel Physical Therapist for home health, focusing on evaluating, treating, and planning care for adult and geriatric patients with various diagnoses. The role requires a Wisconsin license and a Master's in Physical Therapy, with a productivity expectation of 4.5 visits per day. Responsibilities

• Evaluate, treat, and plan patient care in accordance with Physical Therapy Standards of practice.
• Document to meet regulatory and reimbursement requirements.
• Meet productivity expectations of an average of 4.5 visits per day.
• Provide care for adult and geriatric patients with orthopedic, neurological, and functional mobility diagnoses.
• Utilize a laptop for point of service documentation and NetSmart EMR for patient records.

Requirements

• Valid Wisconsin license for Physical Therapy.
• Minimum of 1 year of experience in Physical Therapy.
• Master's Degree in Physical Therapy required.
• Experience in home health care.
• Training in working with adult and geriatric populations.
• Knowledge of Physical Therapy standards and practices acquired through a Master's degree in Physical Therapy.
• AHA BLS certification.
