About the position

The Senior Community Care (SCC) role at Optum Home & Community Care involves providing integrated care to senior patients in various settings, including assisted living facilities and their homes. This position requires extensive travel across multiple states to deliver clinical coverage and manage high-risk health plan members, ensuring they receive comprehensive medical and behavioral care. The role is part of an interdisciplinary team focused on improving access to care and enhancing the health journey of patients.

Responsibilities

• Provide clinical coverage for the primary APC during periods of PTO or short/long term absences
• Manage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, and interventions
• Manage members during a short term, sub-acute rehabilitation episode
• Conduct comprehensive, monthly/routine, and/or acute care visit assessments
• Effectively manage acute and chronic medical and behavioral conditions
• Ensure accurate and complete documentation, including ICD 10 conditions
• Communicate and collaborate with the interdisciplinary care team

Requirements

• Active NP or PA license or ability to obtain by start date, unencumbered and free of any open/unresolved disciplinary actions
• For NP: Active ANCC or AANP national certification in Family, Adult, Adult-Gerontology Primary Care, etc.
• Current, active DEA licensure/prescriptive authority or eligibility to obtain post-hire, per state regulations
• 1+ years of clinical/family/geriatric practice experience as an NP or PA
• Willingness to travel via car, airplane, etc.
• Willingness to obtain additional licensure in assigned geographic areas
• Ability to complete physical requirements of the job including lifting a 30-pound bag, navigating stairs, and performing physical assessments
• Proficient with electronic medical records and the internet
