About the position

The Treatment Coordinator of Services at Communities For People, Inc. is responsible for facilitating service delivery to assigned caseloads, primarily focusing on youth and families involved with the Department of Children and Families (DCF). This role involves case management, developing treatment plans, and ensuring quality service provision through collaboration with families and service providers. The position offers a hybrid work model and emphasizes professional development and making a positive impact in the community.

Responsibilities

  • Case management duties for assigned caseload.
  • Coordinating and facilitating meetings for families receiving home-based and/or placement services.
  • Developing treatment plans based on family meetings.
  • Attending meetings as assigned, which may include traveling around the state.
  • Completing all necessary paperwork efficiently and in a timely manner.
  • Proficient in report writing and computer skills.
  • Working collaboratively to ensure quality service provision.

Requirements

  • Bachelor's degree in Social Work or related field.
  • Work experience with DCF involved families, provider community, and/or related service provider.
  • Knowledge of DCF practices and policies preferred.
  • Must have a valid driver's license and transportation.

Nice-to-haves
