The Triage Care Communication Manager will focus on the development and management of the centralized triage teams and programs to support the continuous improvement of overall care and communication delivery to patients and care teams throughout the organization. This role is pivotal in managing the team responsible for triage and collaborating with various departments to enhance communication activities within the organization. The Triage Care Communication Manager will work closely with the Patient Services Director and support Care Transformation strategies, ensuring that patient care is both innovative and efficient. In this position, the manager will be responsible for developing and implementing communication workflow processes, protocols, policies, and procedures that facilitate patient-centric care. This includes leading a collaborative, cross-departmental environment to ensure that care delivery is scalable and focused on patient safety. The manager will assist in the continuous improvement process by working with organizational leadership to identify areas of excellence and opportunities for enhancement. The role also involves evaluating the effectiveness of team productivity by measuring and reporting on key performance indicators, benchmarks, and metrics. The Triage Care Communication Manager will provide leadership and oversight for project management activities that support company-wide Care Communications, ensuring compliance with HR policies and accountability for achieving departmental goals. Additionally, the manager will serve as a subject matter expert on systems used by the Triage Care Communication team, such as patient engagement platforms and electronic medical records (EMR).
