About the position

As a Triage Registered Nurse at Heart to Heart Hospice, you will play a crucial role in providing telephonic triage to hospice patients and their families during after-hours, weekends, or at other times when the office is closed. This position requires a compassionate and skilled nurse who is dedicated to enhancing the lives of patients with life-limiting illnesses. You will be responsible for ensuring that after-hour visits are scheduled with the on-call team in a timely manner, and that all documentation is completed and submitted to the site at the end of each shift to ensure continuity of care for the weekday team. Additionally, you will manage referrals that come in over the weekend and after-hours, providing education and support to patients and families/caregivers as needed. In this role, you will also conduct patient visits as necessary, which may include admissions, routine check-ins, pronouncements, and tuck-ins. The ideal candidate will have a strong background in nursing, particularly in a healthcare or clinical setting, and will be eager to join a team that is committed to making a difference in the lives of others. Responsibilities

  • Provide telephonic triage to hospice patients and their families during after-hours, weekends, or at other times when the office is closed.

Requirements

  • Current State Registered Nurse License
  • Current Driver's License & Reliable Transportation
  • Two years of experience as an RN preferred in a healthcare/clinical setting
