Responsibilities
- Independently perform home visits to conduct functional, psychosocial and environmental safety assessments.
- Determine eligibility of enrollees.
- Establish an individual plan of care that enables members to remain safely at home despite long-term care needs.
- Listen and communicate with a culturally diverse population including caregivers and other providers.
- Utilize time management skills effectively.
Requirements
- Current NYS RN license and registration
- Minimum of one year experience in acute care or community health nursing
- Working knowledge of Medicare, Medicaid and NYS DOH standards and regulations
Nice-to-haves
- Bachelors of Nursing degree preferred
- Bilingual English/Spanish or English/Chinese desirable
