About the position

The Clinical Assessment Manager - Care Delivery (RN) position is focused on providing comprehensive clinical assessments for the elderly population within the PACE (Program of All-Inclusive Care for the Elderly) framework. This role involves conducting in-home assessments, ensuring compliance with UAS-NY regulations, and collaborating with various teams to facilitate effective care delivery. The position requires a strong commitment to quality improvement and the ability to work independently in a field setting.

Responsibilities

  • Conduct in-home assessments for PACE participants utilizing the UAS-NY to determine eligibility for Community Based Long Term Care Services.
  • Evaluate and review previous assessments to ensure accurate documentation of changes in condition and care needs.
  • Promote and educate potential referral sources about the PACE program and its benefits.
  • Document all interactions and outcomes in the designated clinical management/electronic record system.
  • Work closely with intake and enrollment teams regarding assessment appointments.
  • Complete all assessments, notes, service plans, and documentation timely (within 48 hours).
  • Maintain knowledge of applicable CMS/PACE guidelines and organizational policies.
  • Engage in continuous quality improvement activities to enhance staff performance and participant outcomes.

Requirements

  • Bachelor's degree in Nursing or a related health field (preferred).
  • Minimum of one year of experience working with a frail or elderly population.
  • Three to five years of experience in managed care programs such as MLTC, LTHHCP, MAP, or PACE.
  • Five or more years of experience in care/case management, disease management, or quality assurance.
  • Current active and unrestricted RN license in New York State.
  • Proficiency in utilizing and analyzing UAS-NY and Lenavi.

Nice-to-haves

  • Bilingual skills in Spanish, Korean, Chinese, or Russian are preferred.
  • Experience with Customer Relationship Management platforms and clinical management systems.
