About the position

The Clinical Assessment Manager - Care Delivery (RN) position focuses on providing comprehensive clinical assessments for the elderly population enrolled in the PACE program. This role involves conducting in-home assessments, ensuring compliance with health regulations, and collaborating with various teams to facilitate effective care delivery. The position requires a strong commitment to improving the health outcomes of participants through thorough evaluations and documentation.

Responsibilities

• Conduct in-home assessments for PACE participants utilizing the UAS-NY to determine eligibility for Community Based Long Term Care Services.
• Evaluate and review previous assessments to ensure accurate documentation of changes in the electronic medical record (EMR).
• Promote and educate potential referral sources about the PACE program to encourage participation.
• Develop proposed service plans and obtain necessary documentation during home visits.
• Document all interactions and outcomes in the designated clinical management/electronic record system.
• Work with the quality review team to ensure the accuracy and completeness of all necessary documentation.
• Maintain knowledge of applicable CMS/PACE guidelines and relevant organizational policies.
• Engage in continuous quality improvement activities to promote optimal staff performance and participant outcomes.

Requirements

• Bachelor's degree in Nursing or a related health field.
• Current active and unrestricted NYS Registered Nurse license.
• Minimum of one year of experience working with a frail or elderly population.
• Three to five years of experience in managed care programs.
• Five or more years of experience in care/case management or related fields.
• Proficiency in utilizing and analyzing UAS-NY and Lenavi.

Nice-to-haves

• Bilingual skills in Spanish, Korean, Chinese, or Russian are preferred.
• Experience with Customer Relationship Management platforms and clinical management systems.
