About the position

Amazing Home Care is seeking motivated and experienced Registered Nurses (RNs) to provide home health care services in the Bronx, NY. The role involves performing patient visits, coordinating care with Home Health Aides (HHAs), and ensuring the delivery of quality patient care under physician orders. The position is open to both part-time and full-time candidates, including new graduates.

Responsibilities

• Admit and reassess home health patients according to physician orders.
• Coordinate and supervise Home Health Aides in patient care delivery and discharge.
• Initiate home care visits and regularly reassess patient needs and progress.
• Develop and implement the plan of care under doctor's orders.
• Observe, assess, and document all patient progress.
• Notify physicians and other personnel of changes in patient conditions.
• Educate patients and caregivers on disease processes, medications, and treatment plans.

Requirements

• Active New York State RN License.
• Vaccination against COVID-19.
• 1+ years of experience in a Hospital & Home Care setting.
• Excellent communication and time-management skills.
• Ability to travel to multiple patients per day.

Nice-to-haves

• Bilingual in English & Spanish/Creole/Cantonese/Mandarin/Russian.
• UAS Experience preferred.
