This role may be worked as either a 4- or 5-day per week schedule.

Responsibilities

  • Conduct comprehensive assessments using the NYS Department of Health Universal Assessment System (UAS).
  • Perform both in-person and telephonic assessments to evaluate clinical, social, and individual needs.
  • Create tailored care plans based on assessment findings.
  • Collaborate with the assessment scheduling team to confirm assessment sessions.
  • Engage with Members, families, and providers to gather crucial information.
  • Review care plans and provide essential self-care training.
  • Document assessments promptly and work closely with the UAS Nurse Reviewer.
  • Participate in quality improvement initiatives to enhance service delivery.

Requirements

  • Minimum of an Associate's degree in nursing.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Active NYS RN License and valid NYS identification.
  • At least two years of clinical experience, ideally in home health care or within a health plan.
  • Familiarity with NYS Managed Long Term Care (MLTC) and Medicaid/Medicare plans.
  • Proficiency in the NYS Department of Health's Uniform Assessment System (NYS-UAS) preferred.
  • Strong analytical and problem-solving skills.
  • Proficiency in standard office software applications like MS Word, Excel, Outlook, and PowerPoint.
  • Familiarity with electronic health records systems.
  • Ability to communicate in languages spoken by VCMAX Members, such as Spanish or Mandarin.
