About the position
The LPN Unit Care Coordinator is responsible for supervising and managing patient care within a healthcare unit, ensuring compliance with laws and regulations while providing leadership and support to nursing staff and CNAs. This role emphasizes effective communication with patients and families, as well as coordination of care to promote the well-being of patients.
Responsibilities
- Supervise and direct the daily functions of unit nurses and CNAs.
- Chart patient information accurately and in a timely manner.
- Manage and coordinate patient care services through interpersonal contact.
- Prepare and administer medication as ordered by a physician.
- Respond to emergency situations related to patient and staff safety in a leadership capacity.
- Coordinate patient care plans and services with Registered Nurses (RNs).
- Exhibit excellent customer service and maintain a positive attitude towards patients.
- Assist in the evacuation of patients when necessary.
- Demonstrate dependable and regular attendance.
- Communicate effectively and function productively on an interdisciplinary team.
Requirements
- Attended an accredited LPN program.
- Currently licensed/registered in applicable State as a Licensed Practical/Vocational Nurse (LPN/LVN).
- One (1) year of geriatric nursing experience preferred.
- CPR certification upon hire or obtain during orientation, with maintenance of current certification.

- Advanced knowledge in the field of practice.
- Ability to make independent decisions when circumstances warrant.
- Knowledge of laws, regulations, and guidelines governing functions in post-acute care facilities.
