The position is responsible for the comprehensive planning, identification, and direction of all phases of resident health care within a designated residential unit for a 24-hour period. This role encompasses total resident health care, unit management, and ensuring a conducive unit environment. The individual will oversee the day-to-day administration of the unit, ensuring that resident health care is effectively directed and managed. This includes completing Minimum Data Set (MDS) assessments and care plans, which are crucial for documenting resident needs and care strategies. The MDS assessments are entered into the computer system, and the individual will utilize Equip reports and MX Manage reports to assist in this process. Communication is key, as the position requires relaying assessment and care plan information to the nursing staff and utilizing the CareTracker system to obtain necessary resident information. The role also involves attending team conferences, assisting residents with restorative care and self-help programs to promote independence, and making rounds with physicians to ensure that all physician orders are current and appropriately communicated to nursing personnel. The individual will be responsible for planning and delegating assignments to nursing staff, consulting with the Assistant Director of Nursing to secure necessary human and material resources, and conducting unit meetings to keep staff informed of any changes in facility policies and procedures. In addition to these responsibilities, the position requires conducting in-depth resident assessments and ensuring that care plans are developed, reviewed, and revised as necessary. The individual will provide feedback to staff regarding their performance and evaluate the performance of all unit nursing personnel at least annually. The role also includes admitting and orienting new residents to the unit, requesting supplies and equipment as needed, and overseeing direct care activities to ensure compliance with facility policies. Monitoring residents during meals, conferring with dietitians, and documenting and investigating accidents and incidents are also part of the job. The individual will be responsible for ensuring adherence to infection control practices and may participate in committees as necessary. The position requires frequent rounds on the unit to evaluate resident status and staff performance, checking for skin problems, and overseeing treatment plans. The individual must also assist with the orientation and supervision of other resident care personnel and maintain adequate staffing for the unit. Availability to work beyond standard hours may be required, along with other related duties as directed by supervisors or administration.
