Responsibilities

• Evaluate the patient and provide treatments as per the plan of care.
• Implement the plan of care and provide accurate and timely documentation.
• Educate patients and their families/caregivers about diagnoses and care plans.
• Coordinate home health services and report identified needs to the clinical team.
• Work closely with the Clinical Management Team to ensure quality patient care.

Requirements

• Bilingual in Spanish and English.
• Current California-issued RN License.
• CPR Certification.
• BLS Certification.
• Driver's License.
• At least 1 year of nursing experience.

Nice-to-haves
