About the position

The Home Health Registered Nurse (RN) is responsible for providing nursing care to patients of all ages in their homes. This role involves coordinating care with an interdisciplinary team, patients, families, and referring agencies, while ensuring quality and continuity of care through comprehensive assessments and treatment plans.

Responsibilities

• Administer nursing care for patients in their place of residence.
• Conduct initial and ongoing comprehensive assessments of patients' needs.
• Implement and document the plan of care, ensuring quality and continuity of care.
• Educate patients and families on self-care techniques and nutritional needs.
• Determine the scope and frequency of services based on patient needs.
• Assess caregiver capabilities to meet patient needs.
• Initiate preventative and rehabilitative nursing procedures.
• Apply infection control and standard precautions in patient care activities.
• Provide clinical direction to Home Health Aides and LPNs/LVNs.
• Prepare clinical and progress notes to demonstrate patient progress.
• Communicate effectively with patients, families, and healthcare professionals.

Requirements

• Current RN License
• Valid Driver's License
• CPR Certification
• Valid Auto Insurance
• At least 2 years of Home Health RN experience
• HomeCare HomeBase experience preferred
