About the position

The Veteran Directed Care Program Case Manager is responsible for providing case management services to veterans enrolled in the Veteran Directed Care (VDC) program. This role involves conducting assessments, maintaining service agreements, and ensuring compliance with program expectations while supporting veterans in their independent living goals.

Responsibilities

• Schedule appointments with veterans and surrogates (if applicable).
• Travel throughout designated area of Southeastern Mass., Cape Cod and the Islands.
• Complete Initial, Quarterly and Annual Assessments (in-person & telephonically).
• Develop and maintain Service & Task agreements with the veteran and surrogate, if applicable.
• Provide skills training to VDC veterans and surrogates in accordance with Veteran Affairs (VA) and IA rules and regulations.
• Maintain the veteran's monthly Spending Plan, to reconcile the VA issued budget.
• Assure and maintain strict confidentiality and professional behavior in dealing with veterans & their families as well as colleagues.
• Maintain accurate up-to-date files and documentation of all contacts, management sessions and topics, and any other record keeping duties as required.
• Participate in agency staff meetings, VDC meetings, Team meetings, and other external meetings as required.
• Other VDC Program duties, as required.

Requirements

• Bachelor's Degree with one year experience or five years of experience in Social Services.
• Good interpersonal and organizational skills.
• Strong time management skills.
• Reliable transportation and willingness to travel.
• Legally registered and insured vehicle required.
• High school diploma or equivalency.
