About the position

The Veteran Program Housing Case Manager at St. Patrick Center is responsible for providing comprehensive case management services to veterans in need of permanent housing. Responsibilities

  • Provide wrap-around case management services for veterans in need of permanent housing.
  • Assist clients with immediate housing needs and connect them to community resources.
  • Support clients in developing life skills and achieving financial stability.
  • Conduct crisis intervention as necessary to ensure client well-being.
  • Utilize best practices to increase overall client stability.

Requirements

  • Experience in case management or social work, particularly with veterans or similar populations.
  • Strong understanding of housing resources and community services.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively with clients and other service providers.

Nice-to-haves

  • Familiarity with the Grant Per Diem program.
  • Knowledge of mental health and substance abuse issues affecting veterans.
  • Experience in crisis intervention techniques.
