About the position

The Veteran Service Care Coordinator plays a crucial role in the healthcare team, acting as a liaison between Marshfield Clinic Health System, Veterans Affairs, and the community. This position focuses on facilitating access to services for Veterans, screening for unmet medical needs, and providing support to optimize care coordination and improve clinical outcomes.

Responsibilities

• Serve as a liaison among Marshfield Clinic Health System, Veterans Affairs, social services, and the community to facilitate access to services for Veterans.
• Assist with screening Veterans for unmet medical needs, social determinants of health, clinical quality indicators, and mental health conditions.
• Provide basic clinical, clerical, and technical support to assist Veteran patients.

Requirements

• High school diploma or equivalent.
• Completion of courses in Medical Terminology, Anatomy & Physiology, and Pharmacology.
• Minimum of one-year experience in customer service within human services, healthcare, community outreach, or health education.
• Proficiency with written and verbal communication skills and Microsoft Office Suite.

Nice-to-haves

• Graduate of an accredited Medical Assistant program with AAMA or RMA certification.
• Associate degree in wellness, health promotion, health education, or related field.
• Bilingual capabilities.
• Two years' experience in health and wellness or related field.
• Experience working with the needs of the Veteran population.
• Military service.
