About the position
The Veteran Service Care Coordinator plays a crucial role in the healthcare team, serving as a liaison between the Marshfield Clinic Health System, Veterans Affairs, and the community. This position focuses on facilitating access to services for Veterans, screening for unmet medical needs, and optimizing care coordination to improve clinical outcomes. The coordinator provides essential clinical, clerical, and technical support to Veteran patients, ensuring their needs are met effectively.
Responsibilities
- Serve as a liaison among Marshfield Clinic Health System, Veterans Affairs, social services, and the community.
- Assist with screening Veterans for unmet medical needs and social determinants of health.
- Provide basic clinical, clerical, and technical support to assist Veteran patients.
- Facilitate access to services for Veterans to optimize care coordination.
- Improve clinical outcomes for Veterans through effective communication and support.
Requirements
- High school diploma or equivalent required.
- Completion of courses in Medical Terminology and Anatomy & Physiology or equivalent military training.
- Minimum of one-year experience in customer service within human services, healthcare, or community outreach.
- Proficiency in written and verbal communication skills and Microsoft Office Suite.
Nice-to-haves
- Graduate of an accredited Medical Assistant program with AAMA or RMA certification.
- Associate degree in wellness, health promotion, health education, or related field.
- Bilingual capabilities.
- Two years' experience in health and wellness or related field.
- Experience working with the needs of the Veteran population.
- Military service.
