About the position

The CxC Virtual Care Nurse at AdventHealth is responsible for coordinating care, educating clients, and providing protocol-driven interventions to improve clinical and whole health outcomes. This role involves virtual assessments, collaboration with healthcare providers, and the development of care plans, all while ensuring exceptional customer service and adherence to established workflows.

Responsibilities

• Coordinate care and educate clients on health needs.
• Provide virtual assessments and develop care plans based on protocols.
• Collaborate with healthcare providers to ensure comprehensive care.
• Document all interactions and maintain accurate consumer records.
• Provide referrals to healthcare providers and community resources as needed.
• Utilize critical thinking to triage symptomatic consumers and escalate when necessary.
• Adhere to established workflows and department protocols.
• Maintain knowledge of AdventHealth services and offerings.

Requirements

• Effective communication skills in English, both orally and in writing.
• Ability to type at a minimum speed of 45 words per minute.
• Proficiency in Microsoft Suite and Adobe programs.
• Technological aptitude for CRM, telephony, and electronic health records.
• Associate of Science degree in Nursing from an accredited school.
• Minimum of 2 years of clinical experience as a Registered Nurse.
• Multistate licensure as a Registered Nurse (RN).
