The Virtual Care Nurse (LPN) at Concierge Home Care plays a crucial role in supporting the Complex Care Team Program by providing virtual care to high-risk patients. This position involves collaborating with the clinical team to monitor patients, facilitate their needs, and educate them on medical conditions to prevent hospitalizations. The role is designed for a licensed practical nurse who is skilled in telehealth and patient assessment, ensuring comprehensive care delivery in a virtual environment.

Responsibilities

• Perform virtual visits and assess psychosocial needs of patients.
• Monitor telehealth and facilitate patient needs across the continuum of care.
• Provide education concerning relevant medical diagnoses and factors that increase hospitalization risk.
• Track data and identify hospital admission trends.
• Attend rehospitalization-related meetings, conferences, and symposiums.
• Assist with the innovation and implementation of protocols and guidelines for best practices in care.
• Manage virtual care and telehealth systems by providing assessments and education.

Requirements

• Unencumbered Florida Licensed Practical Nurse (LPN) license required.
• Minimum of two years of experience as a home health clinician.
• Strong clinical background in patient monitoring and triage.
• Experience in preventing patient hospitalizations is required.
• Proficiency in Microsoft Office is preferred.
