About the position

CHI Health Clinic is seeking a dedicated Virtual Clinic RN to join our team, focusing on hospital discharge calls for approximately six primary care clinics across Nebraska. This role is pivotal in providing virtual patient care, ensuring that patients receive the necessary support and information following their hospital discharge. The Virtual Clinic RN will be responsible for accepting inbound communications from patients, pharmacies, and other healthcare entities, as well as performing outbound calls to notify patients of lab and imaging results. This position requires a strong understanding of practice guidelines, standards of care, and federal and state regulations to ensure compliance and quality care delivery. The Virtual Clinic RN will engage in telephone triage, utilizing evidence-based protocols to assess patient needs and provide appropriate guidance. Education is a key component of this role, as the RN will educate patients and their families about health maintenance and the management of both acute and chronic conditions. The RN will also follow medication refill protocols and ensure accurate documentation of patient interactions. This position is designed for individuals who are committed to providing high-quality care and support to patients in a virtual setting. This role requires the RN to live within a 50-mile radius of Omaha, NE, as in-person training and meetings are mandatory. The working hours are Monday through Friday, from 8 AM to 5 PM, with no weekends, holidays, or on-call duties. Responsibilities

  • Provides virtual patient care based on practice guidelines, standards of care, and federal/state laws and regulations.
  • Accepts inbound communication such as calls from patients, pharmacies, and other healthcare entities.
  • Performs outbound calls to notify patients of reviewed lab and imaging results according to protocols that require review of the patient chart to answer questions.
  • Performs telephone triage utilizing evidence-based triage protocols.
  • Educates patients and families about health status, health maintenance, and management of acute and chronic conditions.
  • Follows medication refill protocols and acts accordingly to those protocols.
  • Ensures accurate patient documentation.
  • Performs other duties within scope of role as assigned.

Requirements

  • Current Registered Nurse (RN) license that allows you to practice in our state.
  • Graduate of an accredited nursing program.
  • Basic Life Support (BLS) from the American Heart Association required.
