About the position

The Virtual Registered Nurse (RN) at Albany Memorial Hospital HUB is responsible for collaborating and facilitating optimal patient care through digital platforms. This role involves managing the patient's care plan from admission to discharge, utilizing clinical knowledge and critical thinking skills to ensure effective transitions of care. The Virtual RN will also monitor quality metrics and ensure accurate documentation in patient records while performing various roles such as mentor, clinician, educator, and consultant.

Responsibilities

• Collaborate and communicate effectively to provide optimal patient care through digital platforms.
• Assess, plan, implement, coordinate, monitor, and evaluate the patient's plan of care from admission to discharge.
• Utilize clinical knowledge and critical thinking skills to support the plan of care.
• Ensure seamless transitions of care across the continuum.
• Monitor quality metrics specific to the department and ensure accurate documentation in patient records.
• Perform roles of mentor, clinician, educator, consultant/collaborator, and navigator.

Requirements

• Valid New York State Nursing License from an accredited professional School of Nursing.
• Minimum of 8 years of experience in healthcare/critical care as a Registered Nurse.
• Experience with Electronic Medical Records.
• Current or recent bedside RN experience.

Nice-to-haves

• Bachelor of Science in Nursing (BSN).
